I have had dermal melasma for over 10 years. Like many of you I have tried many products. Not laser though because I am scared it will make the condition worse.
Recently I bought some Meladerm and wanted to do anything I could to make sure that it gets deeper into my skin where the pigment is sitting. The problem with dermal is that product cannot get to the dermal layers of the skin.But I think I have found something that is helping me.
When I was at my Derms 2 weeks ago I noticed a product there which is like a roller - with needles on it. Very very fine ones. They are just called skin rollers, or the process is called skin needling.
The rollers have 150+ tiny needles on them which you roll on your face. I use .5mm - it is not painful although a little uncomfortable. The idea behind the rollers is that the tiny needles make channels into your skin so that it can absorb skin care products better. Why I think it is good for dermal melasma is that it can help the products we buy to get to the deeper levels of your skin much more effectively than the creams just sitting on the surface.
I often read threads and reviews about products and five out of six of the reviews women will say the product didn't work for them - yet it can work for many others. Perhaps this is because the products are not getting deep into their skin.
Anyway... to cut a long story short. This is working for me. The products I use are finally getting to the dermal layers - so it's like I am treating epidermal melasma. After using this for 10 days with my skin care products (I am using others as well as Meladerm) my melasma is breaking up and ligtening - it's becoming more like freckles than big lumps of dark patches.
If you want to get them I know you can buy them on the net - but be careful, alot of companies sell them and some are REALLY expensive - others you can get for cheaper.
I would suggest you just search the net for 'skin needling' and see what I mean - it's been around for many years. I saw that there is an article about Angelina Jolie - apparently she uses it. Probably not for melasma though

I don't want to advise you of any particular supplier because I know it is not allowed here and this is totally not a promotion for any particular roller. there are thousands of them out there and some are very cheap (i.e. $10 US).
I also think that you need to treat melasma from the inside too so that your body is not producing more and more pigment.
I would love this to work for others. I know how awful melasma is.
