Help! Painful butt acne!

Please do not judge me/think I am unclean as I am probably the biggest clean/hygiene freak out there!! I change my underwear daily, make sure to change if it's hot/sweating, obviously scrub in shower, etc. Ever since puberty, I am getting pimples on my butt! They are often big and painful. I found out it's genetic and my dad also gets them to this day. I have tried nearly everything... now I use neutrogena anti-acne face wash on my butt (lol) and this keeps them at bay for the MOST part but I still get a painful big one every month like so::

Honestly I have tried everything, this is very embarrassing and I envy women with nice, smooth bums! I think I might have to go to a derm and shell out some serious $$$$ if I am unable to clear it up myself... Any advice??

Sweetie, the derm would be the very best place to start. You may not necessarily have to shell out a ton, but you do want the peace of mind of knowing what it is exactly, so that you can figure out what the line of treatment will be and what your options are.

You are right, thank you. I popped this one and am all cleared up there for now but I know it is a matter of time before it happens again :(. I will definitely need to go to a doctor/derm as I have dealt with this for years. My mom says it's because I don't pop them as soon as I feel one coming in but i feel that is just bad advice..

You aren't dirty. This is actually called folliculitis. Please do not touch or mess with these bumps any longer, this will only cause scarring and more hyperpigmentation. Stop scrubbing this area when you shower because it will only cause more irritation. Also be cautious of what soap you use because this can also be the issue. What you need is a good exfoliating body wash like glytone. I recommend not scrubbing the product in and just gently washing the area with it. The gylcolic acid will do a lot for your skin and prevent new bumps from forming as well as helping get rid of the discoloration. Make sure to pair this with a good AHA lotion which will also help in the exfoliating process. Please be gentle with your skin because from what I see your skin gets discolorations from injury. Once you heal your skin and you have it under control, you can look into more methods of lightening the pih marks through gentle chemical peels. I prefer lactic acid for this. Please do a lot of research on this advice and if it doesn't help, then try the derm. The dark marks will probably take a long time to clear up even after you get the bumps under control so have patience.
